Alphabet’s Verily unveils AI-powered chronic care solution – Lightpath

Alphabet's precision health subsidiary Verily announced Lightpath, a next-generation chronic care solution designed to address the evolving needs of health plans and their members.

The initial offering, called Lightpath Metabolic, focuses on empowering individuals with cardiometabolic conditions to achieve and maintain their health goals. Open enrollment is targeted for early 2026.

How does it work?

Lightpath Metabolic leverages the clinical, data science, and user experience expertise that has driven the success of Verily's existing solution - Onduo. However, Lightpath offers a more comprehensive and adaptable approach.

The solution provides a suite of modular programs, allowing for a flexible care plan tailored to each member's specific needs. The program options include:

  • Metabolic Intensive: Designed for individuals managing type 1 or 2 diabetes, hypertension, comorbid obesity, and/or hyperlipidemia.
  • Weight Loss Intensive: A clinically-guided program featuring the use of approved anti-obesity medications.
  • Metabolic Improvement: Focuses on diabetes prevention, hypertension management, and well-controlled metabolic conditions.
  • Metabolic Achievement: Provides long-term monitoring and support for members who have successfully achieved their health goals.

Lightpath is built upon Verily's robust precision health platform, which integrates data from various sources — including health records, connected devices, and other healthcare interactions. By analyzing this data, Lightpath delivers personalized care plans, recommendations, and motivational nudges to keep members engaged.

The program pairs this cutting-edge technology with a team of healthcare professionals, including endocrinologists, pharmacists, primary care physicians, nurses, and registered dietitians. As clinically indicated, members can access care through telemedicine visits and benefit from medication review and optimization.

Furthermore, Lightpath incorporates recent advancements in artificial intelligence to support both members and clinicians. Members will have access to an AI assistant that personalizes their care plan, monitors behavior patterns, and promotes continued engagement. On the other hand, clinical staff will benefit from an AI-powered assistant designed to streamline workflows.

Cardiometabolic conditions are a major concern for health plans and their members, impacting quality of life, productivity, and healthcare costs.

Lightpath aims to address these challenges by providing personalized care plans that empower individuals to proactively manage their health, ultimately improving health outcomes while reducing healthcare costs.

The context

Since 2015, Onduo has provided personalized virtual care management to individuals living with diabetes and other chronic conditions. A peer-reviewed study demonstrated significant improvements in glycemic control and other health markers for type 2 diabetes patients using Onduo's chronic care model.

Verily plans to offer Onduo users the option to migrate to Lightpath Metabolic in late 2025.
